The Future Of Opioid Analgesics

چکیده
PROLOGUE The opioid analgesics represent an important class of agents for the treatment of severe clinical pain, but many effects often limit their usefulness. This manuscript will discuss recent findings in opioid pharmacology, and how medicinal chemists may be able to utilize these findings of novel opioid analgesics with reduced undesired effects. As such, the manuscript serves as an example of integrating medicinal chemistry, pharmaceutical chemistry, and pharmacology, and relating them to the clinical problems associated with effective pain management.

Clinical implications of opioid pharmacogenetics.
The peer-reviewed literature yields a plethora of examples of variability in patient's responses to medications. The rapidly progressing field of pharmacogenetics offers insight into the variation in responses observed clinically, and in particular for the variability observed among patients administered mu opioid analgesics. Genetic variation leads to interperson variability in drug absorption...
متن کاملOpioid pharmaceuticals and addiction: the issues, and research directions seeking solutions.
There are few pharmaceuticals superior to opiates for the treatment of pain. However, with concerns of addiction, withdrawal and questionable efficacy for all types of pain, these compounds are far from a magical panacea for pain-relief. As it is unlikely that other classes of compounds will supersede the opioids in the very near future, it is important to both optimize current opioid therapies...
متن کاملThe changing landscape of opioid prescribing: long-acting and extended-release opioid class-wide Risk Evaluation and Mitigation Strategy
Prescriptions for opioid analgesics to manage moderate-to-severe chronic noncancer pain have increased markedly over the last decade, as have postmarketing reports of adverse events associated with opioids. As an unintentional consequence of greater prescription opioid utilization, there has been the parallel increase in misuse, abuse, and overdose, which are serious risks associated with all o...
متن کاملUnderstanding Risk Factors for Opioid Overdose in Clinical Populations to Inform Treatment and Policy.
Overdoses involving opioid analgesics represent a significant public health problem in the United States. We reviewed the literature on risk factors for overdose, with a focus on studies that examine clinical populations of patients receiving opioids for pain and potential risk factors for overdose in these populations. A structured review resulted in 15 articles published between 2007 and 2015...
متن کاملSecrets of the opium poppy revealed.
Studies concerning drugs of abuse have made major contributions in defining the circuitry, as well as cellular and molecular substrates that underlie certain behaviors. Opiate drugs for example, have revealed important insights concerning pain perception and reward. Up to the late 1960s, opiate drugs were suspected to work by mysteriously perturbing lipid membrane structure. We now know the fol...
